These terms govern your use of iyamedical.com and the online services on it, which are operated by IYA Medical, PLC (“IYA Medical,” “we,” “us”). By using the site you agree to them. If you do not agree, please do not use the site; you can always reach us by phone at 480-771-0000.

The site is information, not medical advice

Content on this site, including descriptions of conditions, procedures, videos and educational material, is provided for general information. It is not medical advice, is not a substitute for an evaluation by a licensed clinician, and does not describe what is right for you. Using the site, submitting a form or chatting with the intake assistant does not create a physician–patient relationship. That relationship begins when you are evaluated by one of our clinicians.

Online intake and the intake assistant

• The intake assistant is automated software that collects the information our front desk needs before your visit. It does not diagnose, treat or recommend care, and anything it says should not be relied on as medical advice.
• Submitting a request is not an appointment. Our scheduling team reviews every request and will contact you to confirm. If you have not heard from us within two business days, please call.
• You agree to provide accurate information and to complete intake and paperwork only for yourself, or for a person you are legally authorized to act for.
• Links to your new patient paperwork are personal to you and expire. Do not forward them. Electronic signatures you provide through our paperwork have the same effect as a handwritten signature.

Descriptions of procedures and patient stories on this site reflect general experience and the experience of the individuals shown. They are not a promise of any particular outcome. Whether a treatment is appropriate for you, and what results you can expect, can only be determined after a consultation.

Acceptable use

You agree not to use the site in a way that is unlawful, that interferes with its operation, that attempts to access accounts, links or records that are not yours, or that submits false information or automated requests.

Our content

The text, images, videos, logos and design of this site belong to IYA Medical or are used with permission. You may view and print pages for your personal, non-commercial use. Any other use requires our written permission.

Third-party sites and services

The site links to and embeds services operated by others, such as our patient portal, video platforms and maps. We do not control those services and are not responsible for their content or their handling of your information.

Disclaimer and limitation of liability

The site is provided “as is.” To the fullest extent permitted by law, we disclaim all warranties about the site, and we are not liable for indirect, incidental or consequential damages arising from your use of the site or your reliance on its content. Nothing in these terms limits our responsibilities to you as a healthcare provider under applicable law, or any right you have under our Notice of Privacy Practices.

Governing law

These terms are governed by the laws of the State of Arizona. Any dispute about the site will be brought in the state or federal courts located in Maricopa County, Arizona.

Changes

We may update these terms by posting a new version here with a new effective date. Continued use of the site after a change means you accept the updated terms.
