Understanding Dialysis Access
Hemodialysis requires access to the bloodstream that can handle the high blood flow rates needed for effective treatment. There are three main types of dialysis access:
Arteriovenous Fistula (AV Fistula)
Created surgically by connecting an artery to a vein, usually in the arm. Considered the "gold standard" for dialysis access due to longevity and lower infection rates.
Arteriovenous Graft (AV Graft)
A synthetic tube connecting an artery to a vein when a fistula isn't possible. Can be used sooner than a fistula but may have more complications.
Dialysis Catheter
A tube placed in a large vein, usually in the neck, chest, or groin. Used for immediate or temporary access but carries higher infection risk.
Interventional Radiology Services
Fistulagram (Dialysis Access Imaging)
Using X-ray and contrast dye, we can evaluate your dialysis access to identify problems like narrowing (stenosis) or clotting.
Angioplasty and Stenting
When stenosis (narrowing) is identified:
- A balloon catheter is used to open the narrowed area
- If needed, a stent may be placed to keep the vessel open
- Restores adequate blood flow for dialysis
Thrombectomy and Thrombolysis
When a fistula or graft becomes clotted:
- Mechanical devices or clot-dissolving medications remove the clot
- Underlying narrowing is treated to prevent re-clotting
- Can often restore function the same day
Catheter Placement and Exchange
When a catheter is needed:
- Tunneled catheters for longer-term use
- Non-tunneled catheters for emergency access
- Catheter exchange when problems occur
Signs Your Access May Need Attention
- Swelling of the access arm
- Weak or absent pulse/thrill in the access
- Difficulty with needles during dialysis
- Prolonged bleeding after dialysis
- Poor dialysis clearance
- Signs of infection (redness, warmth, drainage)
Maintaining Your Access
Tips to protect your dialysis access:
- Keep the access clean and dry
- Feel for the thrill (vibration) daily
- Don't allow blood pressure checks or blood draws from the access arm
- Avoid sleeping on the access arm
- Don't wear tight clothing or jewelry over the access
- Report problems promptly
